DOT Physicals


Sage Chiropractic Clinic provides FMCSA-certified Department of Transportation (DOT) physicals for commercial drivers. Our goal is to help you complete your exam efficiently while meeting all federal requirements, so you can return to driving safely and without unnecessary delays.

To avoid interruptions to your certification, we recommend scheduling your DOT physical approximately one month before your current medical card expires. This allows time to address any health concerns that may require follow-up care or medical clearance from your primary care provider or a specialist.

Our team is committed to providing a professional, respectful, and comfortable experience for every driver. When scheduling your appointment, please let our office know if you have been diagnosed with any medical conditions. Some conditions require additional documentation, and advance notice helps ensure your visit goes smoothly. Most DOT physicals take 15–20 minutes, depending on medical history and required testing.

You’ll have the option to complete DOT paperwork digitally prior to your appointment, or traditional paper forms in the office.

In addition to a paper medical certificate, you will receive a laminated wallet card and online access to your certificate. All required information is submitted electronically—no visit to the DMV is needed.

  • A DOT physical is a federally required medical examination for commercial drivers to determine whether they meet health standards necessary for safe vehicle operation. The exam includes:

    • Vision and hearing screening

    • Blood pressure measurement

    • Urinalysis

    • Comprehensive physical examination

  • A DOT physical is required if you:

    • Hold a Commercial Driver’s License (CDL)

    • Transport hazardous materials

    • Drive a vehicle carrying more than 15 passengers

    • Operate a vehicle with a combined weight over 10,000 pounds

  • DOT physicals must be completed by a certified medical examiner listed on the National Registry of Certified Medical Examiners.

  • To maintain a valid DOT medical card:

    • Schedule your exam before your current card expires

    • Attend regular health checkups

    • Work with your healthcare provider to manage any ongoing conditions

  • Certain conditions—including uncontrolled diabetes, epilepsy, cardiovascular conditions, or vision concerns—may require medical clearance from your primary care provider or specialist. Please inform our office when scheduling your appointment so we can advise you on any necessary documentation.

  • Please bring:

    • A valid driver’s license

    • Glasses or hearing aids, if applicable

    • A list of current medications

    • Relevant medical history
